If you are an eligible same-sex domestic partner interested in applying for coverage under the FLTCIP,
please read important information on how to apply for coverage. Before you submit an application for coverage under the FLTCIP, you will need to provide documentation (the Declaration of Domestic Partnership form) that you and your partner meet the definition of domestic partnership.
