New hires/newly eligible employees only
Information kit and abbreviated underwriting application
# 01 Kit includes educational books and materials as well as an abbreviated underwriting application for new hires/newly eligible employees. To be used only within 60 days of hire/newly eligible status.

After the 60-day period, these individuals can still apply for coverage but must use the full underwriting application.
