Benefits Provided by the Federal Program

Covered Services

You may select one of two coverage options under the Federal Program:

• Facilities-Only
• Comprehensive

Facilities-Only

The Facilities-Only Option provides coverage for services you receive in a nursing home, assisted living
facility, and hospice facility. For the Facilities-Only Option, we will only pay benefits for respite services provided in a nursing home, assisted living facility, or hospice facility.

Comprehensive

The Comprehensive Option provides coverage for all the services covered under the Facilities-Only Option plus services provided by a formal caregiver at home, services provided by an informal caregiver, hospice care at home, and services provided at an adult day care center. For the Comprehensive Option, we will pay for respite services provided by a formal or informal caregiver at home or provided in a facility.

We will only pay benefits for covered services if:

• you are eligible for benefits and, where required, have satisfied the Waiting Period; and
• they are part of a written plan of care approved by us.

Bed reservations, caregiver training, and respite services are covered under both options.
