Benefits Provided by the Federal Program

Maximum Lifetime Benefit

The Benefit Period is the length of time your coverage will last if we pay your Daily Benefit Amount each and every day. You may choose from a 3 Year, 5 Year or Unlimited Benefit Period. If you have a 3 Year or 5 Year Benefit Period, benefits will last longer than the Benefit Period if you do not receive covered services every day or if benefits reimbursed for each day (or, if applicable, week) are less than your Daily Benefit Amount (or, if applicable, Weekly Benefit Amount).

Your Maximum Lifetime Benefit is the total amount of money that we may pay for charges you incur for covered services. It is equal to your Benefit Period (in days) times your Daily Benefit Amount. Your Maximum Lifetime Benefit will be reduced by the amount of benefits that we pay. If you have an Unlimited Benefit Period, your Maximum Lifetime Benefit is also unlimited.
 

Schedule of Benefits

Your Schedule of Benefits that we will send you will show, among other things, the coverage option you select
(Facilities-Only Option or Comprehensive Option), the covered services under that option, the maximum we
will pay for those services per day (or, if applicable, per week), the Benefit Period, and your Maximum
Lifetime Benefit..
