  1. You Initiate a Claim by Calling LTCP at 1-800-582-3337.

  2. Your Care Coordinator Researches Your Claim.

  3. We Notify You of Our Decision.

  4. We Work With You To Develop Your Plan Of Care.

  5. You or Your Provider(s) Send Us Bills for Payment.
