Do I File a Claim?

1. You Initiate a Claim by Calling LTCP at 1-800-582-3337
- If you are unable to perform at least 2 activities of daily living
(i.e. bathing, dressing, eating, transferring, toileting, and continence) for an expected period of at least 90 days due to a loss of functional capacity, or you require substantial supervision due to your severe cognitive impairment, you may be eligible for benefits.
Important: After your Waiting Period, benefits can begin as long as the covered services you file claims for are part of a plan of care developed by a licensed health care practitioner and approved by us.
To initiate a claim, you or any representative
(including a physician or caregiver) may call us.*
When you call, please have the following available:
- Your Social Security number
- The names and telephone numbers of your
physician and other health care providers
- Medical insurance information, including
your insurer’s name and telephone number, if
applicable
We will assign a personal Care Coordinator to help
you with the claims process. You will be able to
contact him or her directly. He or she will stay with
your case throughout the course of your claim.
Your Care Coordinator will ask questions about
your health history, assistance needs, family support,
and related information.
Your Care Coordinator will provide a detailed
explanation of FLTCIP benefits, the claim process,
and related information.
* Note: Although your representative can initiate a claim on
your behalf, we cannot provide him or her with any
information about you or your insurance benefits without
your verbal or written approval or legal documentation
such as a power of attorney.
