2. Your Care Coordinator Researches Your Claim

  • We will ask you to sign a Release of Information Form, releasing your health information so that we can research your claim.


  • Your Care Coordinator will gather information to determine if you are eligible for benefits. This includes contacting your physician and other health care providers involved in your care.


  • We may send a nurse to visit you in your home and/or request a copy of your medical records.
