3. We Notify You of Our Decision

  • Within 5 days of obtaining all required medical information, your Care Coordinator will let you know by phone if we approve or deny your claim. We will send you a letter to confirm the decision.


  • If we approve your claim, we will include with your approval letter an Assignment of Benefits Form and related instructions.


  • If we deny your claim, you have the right to appeal this decision. Your Care Coordinator will explain this process.
