4. We Work With You To Develop Your Plan Of Care

  • Your Plan of Care specifies the types, frequency, provider(s), and cost of care you need, and how much of that cost your insurance will cover.


  • A licensed health care practitioner and/or your care coordinator, working with you, will establish your Plan of Care.


  • Your Care Coordinator will assist with locating services in your area, including any discounted services that might be available.


  • You will determine which provider(s) are right for you. If you have the comprehensive option, this may include informal caregivers such as friends, family members, and other non-licensed caregivers.


  • Your Care Coordinator will work with you to revise your Plan of Care as your needs change.


Note: To avoid using a provider who would not be covered under the FLTCIP, always talk to your Care Coordinator about any changes you plan to make to the services you are receiving before you make those changes.
