4. We Work With You To Develop Your Plan Of Care

• Your plan of care specifies the types, frequency, provider(s), and cost of the care you need, and how much of the cost your insurance will cover.



• You and your Care Coordinator will work together to develop your plan of care.



• Your Care Coordinator will assist you with locating services in your area, including any discounted services that might be available.



• You will determine which provider(s) are right for you. This may include informal caregivers such as friends, family members, and other unlicensed caregivers.



• Your Care Coordinator will work with you to revise your Plan of Care as your needs change.

Note: To avoid using a provider who would not be covered under the FLTCIP, always talk to your Care Coordinator about any changes you plan to make to the services you are receiving before you make those changes.
