5. You or Your Provider(s) Send Us Bills for Payment

  • You can ask us to pay your benefits directly to your care provider. To do so, complete the Assignment of Benefits Form.


  • We will also ask you to provide bills for the services you have received and copies of all Explanation of Benefit (EOB) forms that other insurance companies or Medicare have sent to you, if applicable.


  • If you use a provider who does not have a billing system, such as an informal caregiver, we’ll send you an Informal Caregiver Invoice to submit as a bill.


  • Informal caregivers must provide identification to LTCP. Valid identification includes a copy of their driver’s license or state ID card. A Social Security number is also required for all informal providers.


  • In order to speed up the process, double check that you have signed and dated the Informal Caregiver Invoice after care has been provided. Please mail it to us at the address that is provided on the form. We will pay your approved claim within 5 days of receipt.
