5. You or Your Provider(s) Send Us Bills for Payment

• You can ask us to pay your benefits directly to your care provider. To do so, complete the Assignment of Benefits line on the Assignment of Benefits Form.



• You will need to complete one Assignment of Benefits Form for each provider you want us to pay directly.



• We will also ask you to provide bills for the services you have received and copies of all Explanation of Benefit (EOB) forms that other insurance companies or Medicare have sent to you, if applicable.



• If you use a provider who does not have a billing system, such as an informal caregiver, we’ll send you an Informal Provider Invoice to submit as a bill.



• Informal caregivers must provide identification to LTCP. Valid identification includes a copy of their social security card, driver’s license or state ID card.



• In order to speed up the process, double check that you have signed and dated the Assignment of Benefits Form correctly. Please mail it to us at the address that is provided on the form.We will pay your approved claim within 10 business days of receipt.
