Customizing Your Own Plan

In addition to providing four "pre-packaged" plans, the Federal Program allows you to customize a plan based on your needs.

If you choose to customize a plan, you must make a choice for each of the following five options:

1. Facilities-Only or Comprehensive Option
    
2. Daily Benefit Amount (DBA)
   
   Choose from:

   $50	$75	$100
   $125	$150	$175
   $200	$225	$250
   $275	$300

   Weekly Benefit Amount (Optional) - equal to seven times one of the Daily Benefit Amounts above if you select the Comprehensive Option

   • 3 years
   • 5 years
   • Unlimited
      
4. Waiting Period
   • 30 days
   • 90 days
      
5. Inflation Protection Option
   • Automatic Compound Inflation Option
   • Future Purchase Option
