If you apply for coverage and are approved, you will be eligible for benefits after your coverage becomes effective and when a licensed health care practitioner certifies, and we agree, that:

Benefits can begin (after the waiting period) as long as the covered services are part of a plan of care developed by a licensed health care practitioner and approved by us.

Once you qualify for benefits, you'll need to initiate a claim. Learn more about initiating claims here.
