Before you apply for coverage under the Federal Long Term Care Insurance Program (FLTCIP), you must:


Application Checklist

During the application process, you will need to provide some personal information and answer medical questions related to your health. Depending on the application you use, and the options you choose, you may also need to have the following information available:

  • any prescription medications taken over the past six months
  • name, address, and telephone number of your primary care physician or other health care practitioner
  • name, address, and telephone number of other treating physicians or health care practitioners
  • if you select automatic bank withdrawal as your payment option, you will need a voided check or savings deposit slip (this option is not currently available online; you can request it once you are approved for coverage)
  • if you select payroll or annuity/pension deduction as your payment option, you will need your payroll or annuity office identifier
  • if you select to pay by CSRS/FERS annuity deduction, you will need your annuity claim number
  • a printer to print your application (if you download an application) or the confirmation of your application (if you apply online)

It is important to note that not everyone who applies is approved to enroll in the FLTCIP. Certain medical conditions, or combinations of conditions, will prevent some people from being approved for coverage. Depending on your age and your answers to the questions on your application, you may receive a call from a registered nurse to conduct a telephone interview or schedule an in-home interview. We may also require copies of your medical records.
